Records
Juneau Youth Services’ (JYS) Records Department maintains the personal health information of all the individuals we serve. Records are kept in strict accordance with state and federal law, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA). For more information about your records access rights, please refer to JYS’ Notice of Privacy Practices.
The health records of minors (individuals 18 years of age and younger at the time of discharge) are retained for ten (10) years following discharge or until the individual reaches the age of 25, whichever is longer. Adult health records (individuals 19 years of age and older) are retained for ten (10) years following discharge.
If you would like a copy of your records, complete the JYS Release of Information Form; a link to the form is provided below. Please be sure to complete the form in its entirety; incomplete forms will cause a delay in processing. Print and sign the form then return it to JYS by mail, email or fax. We will accept scanned photos of completed and signed forms, but they must be legible. If you submit your request via fax, please call our Records Department one business day after sending it to make sure it was received.

Most requests are processed within ten (10) business days after JYS receives the completed Release of Information Form. Older records may be in paper form and can take longer to process. If you have questions or need assistance, please contact us.
